Program Goal: To work with individuals, groups, and organizations to improve the delivery of system-wide culturally appropriate services through organizational assessment and development, community assessment and development, individual and organization consultation, and program design, implementation, and evaluation. 

CCCH Organizational Development Model

Phase I: Defining the Assessment Question
  • Identify a steering committee to lead the process internally
  • Develop the assessment goal and 
  • Determine how the assessment data will be used
  • Establish project scope and timeline
Phase II: Internal Assessment
Assess organizational systems and policies based on a comprehensive checklist that includes such items as:
  • Governance Policies
  • Community Partnerships
  • Information Management
  • Human Resources Practices
  • Physical Environment
  • Volunteer Management 
Utilize a variety of assessment methods and tools to measure cultural competency at both the organization and individual level including:
  • CCCH clinical assessment tool based on the CLAS standards
  • Organizational assessment tool that measures where the organization lies on the cultural competency continuum.
  • Individual assessment tool that places the individual on a scale of cultural competency (both perceived level and developmental level).
  • Internal focus groups with staff, board members and volunteers.
Phase III: External Assessment
  • Assess patient/client experiences and opinions of the organization through culturally sensitive satisfaction surveys and patient/client focus groups.
  • Assess how the overall diverse community views the organization through focus groups with community members and interviews with community leaders.
  • Assess how other organizations view the organization through interviews with key organization leaders.
Phase IV: Development Plan
  • Debrief organization as to the assessment findings.
  • Develop goals and objectives for the plan.
  • Partner with organization to develop initiatives and action plans.
  • Prioritize the initiatives based on impact and resources required.
  • Determine the scope of project based on priority and capacity.
  • Develop a timeline and appoint a committee to carry out the initiatives.
Phase V: Implementation
  • Oversee and support implementation of recommendations.
  • Deliver customized training plan.
Phase VI: Evaluation
  • Re-administer applicable assessment tools to evaluate progress towards stated goals.
  • Identify future goals for sustaining organizational change.
